a) Proper selection and processing (cutting and notching) of rigid polystyrene
insulation shall be done by a fabricator which has been licensed by Lite-Form Inc. of
Sioux City Iowa. b) Assembly, placement and bracing of form components shall be done by a company with
previous training and experience in construction of the Lite-Form system. The installer
shall provide evidence of such experience to the architect or engineer.
c) Design of concrete mix and reinforcing shall conform to Can3/-A23.3-M "Design
of Concrete Structures for Buildings".
